Health information
Tai Chi is gentle, but we need to know about anything that could affect how you practise safely, such as joint problems, recent surgery, heart conditions or pregnancy. Health information is "special category data" and gets extra protection.
-
We only ask for what we need to adapt the class for you, and only with your explicit consent.

You can choose not to share it, but we may then be unable to advise you on safe modifications.

It is seen only by your instructor and is never shared, except in an emergency where it is needed to protect your vital interests.
-
Paper forms are kept securely and digital records are password-protected.
